This form will serve as the client’s record in the clinic and can be a reference for their dental history. After passing the form, the patient will tell the front desk staff of the service he wants to avail and then afterwards he will be given the service that he wanted. The client’s history will be based on the forms they filled up and this may serve as the dentist’s reference of the past dental activities of the patient.In this record , the dentist could monitor who are the patients that are active and not. A client could set an appointment for their dental transactions. Telephone is the primary source of communication of the patients to the dental clinic because most patients use telephones to set an appointment or take a schedule with the dentist. Appointments are recorded by writing it in a record book in order for the staffs to remember and make track of the appointment, schedules and activities of the clinic. Before the day of the appointment, the clinic will call the patient to assure the presence of the client in his appointment.

Payments are done through three payment terms. First is the Cash Policy. In this policy , the client who are able to pay the bills of services that he take will pay right after the dental activities are done. Second term is the use of credit cards. The client will show his card and the front desk staff will swipe it to the card swiper/reader and the transaction is done. The third term is the use of health cards. Health cards could come from different companies. It serves as a benefit of the patient from his work that may be helpful.

Nieto Dental Clinic uses a new technology for the different kind ofdental services. In relation with this, anyone could say that the process of dental servicing is reliable. Therefore, as the proponents conducted a study to be able to help our client on their business. We proposed a Computerized Dental Clinic Management System using Credit Card Swipe Reader in order to help them in their daily activites. By the help of this system, the system could be improved for the patients’ satisfactory. Theoretical Framework Database Normalization Normalization is the process of organizing data in a database. This includes creating tables and establishing relationships between those tables according to rules designed both to protect the data and to make the database more flexible by eliminating redundancy and inconsistent dependency.

Redundant data wastes disk space and creates maintenance problems. If data that exists in more than one place must be changed, the data must be changed in exactly the same way in all locations.A customer address change is much easier to implement if that data is stored only in the Customers table and nowhere else in the database. We use this theoretical framework Database Normalization to apply the different normal form: first we apply the first normal form, to not repeat the group of data and to present a reworked data schema where the order put in first normal form. Second is the Second normal form, to improved the presents reworked data schema, but our non-attributes and attributes is not is not part of the primary keys, but it fully dependent in primary keys.Third is the Third Normal form, which our attributes of an entity type are depend on all portions of our primary keys.
